The Toledo Hospital Pharmacy Residency

The Toledo Hospital Pharmacy Residency was established in 2002 as a joint effort between the clinical pharmacists at the family medicine residencies of The Toledo Hospital and Flower Hospital.

Mission

The pharmacy residency exists to train clinical pharmacists to be responsible, competent practitioners who function as part of a health care team to optimize drug therapy/disease state management and patient outcomes in an ambulatory care setting.

The pharmacy residency exists to be competent educators of:
a) Pharmacy residents in the experiental training setting
b) Medical students and resident physicians in pharmacotherapeutics
c) Patients to improve their understanding and use of medications

Curriculum

The flexible, innovative curriculum has been designed to provide experience in:

  • Applying principles of evidence-based medicine to all aspects of one’s career
  • Assisting patients in obtaining medications through Medicare Part D and other patient assistance programs
  • Caring for geriatric patients in extended-care facilities
  • Educating patients, physicians and pharmacy students
  • Hospital care with a primary care team
  • Research in a ambulatory care office setting
  • Using collaborative practice agreements in an office setting to care for patients with diabetes, hyperlipidemia, hypertension and other common disease states 
  • Warfarin management
  • Working with elderly residents of community senior citizen centers

The Toledo Hospital Family Medicine Residency Education: Bachelor of science in pharmacy from Ohio Northern University, master’s degree from The University of Toledo
Responsibilities: Director of pharmacy residency; Rounds with inpatient and geriatric services; coordinates anticoagulation and collaborative practice clinics; research; the use of handheld computers in the residency
Professional Focus: Practical application of evidence-based medicine, collaborative practices

Stephen Thomas, PharmD, BCPS

Flower Hospital Family Medicine Residency Education: Bachelor’s and pharmacy degrees from The University of Toledo
Responsibilities: Precepts pharmacy students from The University of Toledo and the University of Michigan
Professional Focus: Board-certified pharmacotherapy specialist, evidence-based medicine and the application of technology in health care, care of patients with hyperlipidemias, warfarin management.
